A company has been fined more than $17,000 after last year's fatal accident at a work site in Colonie.
OSHA sited JRP Enterprises of Schenectady for three violations stemming from the accident on Nov. 25.
Donald Koons Jr., 32, of Schoharie was killed when he was buried in the collapse of an 11 foot deep excavation on Harmony Court in Colonie.
OSHA's investigation found the project lacked protective systems designed to prevent its collapse. It also failed to have anyone on site who could identify and correct trenching hazards, and they failed to properly train employees to recognize them.
